About the Role

This is apart-time contractor opportunityto joinPlanSmart Proas aSupport Coordinator, workingonsite in our Coolaroo office. You’ll be our first support coordination hire, with the chance tolead and growthis new service offering from the ground up.

We’re looking for someone experienced, connected, and proactive — someone who wants to build their own caseload, develop strong participant relationships, and contribute to business growth, all while working in a supportive office environment.

What You’ll Be Doing

-

Provide tailored support coordination to participants

Coordinate and monitor individualised support plans

Build trusted relationships with participants, families, and providers

Grow your own caseload using your network and our support

Be a key part of launching and shaping our support coordination program

Work from ourCoolaroo office at 1510 Pascoe Vale Road– this is anoffice-based role

Receive commission for referrals and service growth

What We’re Looking For

-

Minimum 3 years’ experience in support coordination or case management

Strong working knowledge of service navigation and plan implementation

A professional network within the disability or care sector

Excellent communication and time management skills

Confidence working independently and making decisions

ABN and appropriate insurances (for contractor role)

Valid Working With Children Check and Police Check

A passion for supporting people and building something meaningful
