Job Description
Cohort Staffing is a highly regarded NDIS-registered provider seeking an experienced Support Coordination Consultant to join their network on a subcontractor basis. This is a unique opportunity for a skilled practitioner to operate independently while benefiting from structured systems, established client referrals, and access to NDIA-managed participants—typically unavailable to independent contractors.
About the Opportunity
• 75% of billable income retained by you – fully transparent and uncapped
• Support clients across all funding types: NDIA-managed, plan-managed, and self-managed
• Work independently with full flexibility – choose your own hours, caseload size, and work style
• Benefit from robust systems, optional admin support, and clear NDIS compliance guidance
• Engage in periodic quality assurance and reporting in line with provider registration requirements – without direct supervision
Role Responsibilities
• Support NDIS participants to understand, implement, and utilise their plans effectively
• Coordinate supports, build capacity, and strengthen informal and formal networks
• Maintain accurate, timely notes and documentation as per NDIS Practice Standards
• Contribute to regular compliance updates (brief summaries provided monthly)
• Operate with a person-centred, goal-focused approach while remaining self-directed
Selection Criteria
• Minimum 1 year of experience in Support Coordination or similar case management
• Deep understanding of NDIS principles, funding streams, and compliance standards
• ABN, professional indemnity and public liability insurance
• Qualifications in allied health, psychology, social work, or community services
• Driver’s licence and reliable transport
• Current (or willingness to obtain) Police Check, NDIS Workers Screening check and WWCC
