We support over 1.6 million people across our region, with a focus on aged care, mental health, Aboriginal and Torres Strait Islander health, digital health, and workforce development.
What You’ll Do
You’ll act as SEMPHN’s frontline, linking GPs, clinics, and other primary care providers with the right tools, resources, and updates. Your day-to-day might look like:

**Be the Go-To Connector:**Building trusted relationships with GPs and primary care providers to make change happen where it counts.
**Championing Change:**Helping providers adapt to new health initiatives and embedding best practices into clinical care.
**Building Capacity:**Coaching and supporting providers to strengthen their business and clinical skills.
**Driving Projects:**Planning, coordinating, and delivering programs like Chronic Disease Management, Palliative Care, and Doctors in Secondary Schools.
**Collaborating Across Teams:**Working with internal experts to ensure reforms are practical, impactful, and provider friendly.
**Solving Problems on the Fly:**Spotting gaps, responding to feedback, and developing solutions that improve care for the community.
About You
You’re experience in the primary health system and are across how providers work!
Have a valid driver’s licence and don’t mind hitting the road to connect.
You’re a people person, easily connecting with everyone you meet building trust, and influencing positive change.
You’re a great communicator, open to feedback, and love working with different perspectives.
You can juggle projects like a pro, keeping on top of timelines, while delivering high-quality outcomes.
You’re a change advocate; comfortable helping others adapt to new systems and processes.
You don’t just identify issues; you jump in and find ways to fix them.
