We’re looking for aClient Intake Coordinatorto guide families through their very first steps with us. You’ll be the welcoming voice on the phone, the reassuring contact by email, and the trusted first connection for people navigating their care options.

What You’ll Do

Be the first point of contactfor new clients and families, making a complex process simple and reassuring

Listen with empathy, assess needs, and guide enquiriestoward the right funding and service pathways

Coordinate onboarding, collecting documents, scheduling assessments, and ensuring a smooth handover

Educate clients and familieson services, funding models, and self-management options in clear, simple language

Represent Sensible Care in the community, building relationships with referral partners and attending events to raise awareness of our services

Support growth and improvementby tracking referral trends and contributing ideas to strengthen the intake journey

What We’re Looking For

-

A natural communicator who enjoys helping people feel at ease

Knowledge of aged care or community services, especially Home Care Packages/Support at Home (advantageous)

Experience in intake, referrals, or client services with a people-first approach

Strong organisational and follow-up skills, with attention to detail

Confidence using digital systems and learning new tools

A passion for client advocacy and values-driven care
