Service Coordinator

Nursing Centred Care Australia (NCCA) is a trusted provider of nursing and community-based supports for NDIS participants, older Australians, and DVA clients. We are seeking an empathetic, organised, and proactiveService Coordinatorto join our team. In this role, you will be responsible for managing client engagement, coordinating service delivery, and maintaining clear and effective communication between participants, families, and care teams.

Key Responsibilities

Coordinate services for NDIS, aged care, and DVA clients.

Be the primary point of contact for participants, families, and external stakeholders.

Match participants with suitable staff based on individual needs and preferences.

Work alongside nurses and care staff to support the implementation of care plans.

Ensure service delivery aligns with participant goals and funding requirements.

Maintain accurate participant records and ensure proper documentation is completed.

Respond to incidents, changes in care needs, and feedback in a timely and professional manner.

Contribute to continuous service improvement and client satisfaction.

About You

-

Experience in service coordination, care management, or a similar role.

Knowledge of NDIS and/or aged care systems and service delivery.

Excellent communication and relationship-building skills.

Strong organisational skills and attention to detail.

Proficient with client management software (e.g., ShiftCare, BrightHR) and basic admin tools.

Empathetic, culturally aware, and client-focused.
