About All Family Nursing Care
All Family Nursing Care is a nurse practitioner-led, registered NDIS provider specialising inmental health, youth support, accommodation, and community nursing. We are passionate about improving the health and wellbeing of individuals, including those from Culturally and Linguistically Diverse (CALD) backgrounds. As part of our commitment to supporting vulnerable members of the community, we now offerspecialised services for children and young people, particularly those who are at risk, vulnerable, or in out-of-home care.
Youth Support Services

Your main responsibilities will include engaging with and supporting Children and Young People who exhibit challenging behaviors, helping them achieve their social, educational, cultural, and emotional goals. You'll also oversee their daily care.
Our aim is to equip Children and Young People with new coping strategies and social behaviors to help them transition to a forever home.
Daily tasks may include:
Providing hands-on support in teaching life skills to our Young People, such as cooking, cleaning, budgeting, completing homework, personal care, booking and attending appointments, participating in sports, and engaging in schooling
Documenting progress notes
Assessing and supporting the developmental progress of our Young People
Liaising with and reporting to management to share information, contribute to planning, and ensure desired outcomes are achieved
Creating activity plans and facilitating community engagement for the Young Person
Safely supporting behaviours using Therapeutic Crisis Intervention (TCI) techniques
School drop-offs and pick ups.

Skilled in building meaningful relationships and achieving positive outcomes
Excellent communicators, able to engage with children with complex needs and liaise effectively with stakeholders
Able to provide consistent emotional support and stability to young people

Requirements

Working with Children Check
Valid Open Driver’s Licence and a reliable, comprehensively insured vehicle
Current CPR and First Aid certificates
Relevant qualificationsin Youth Work, Community Services, or a related field (or currently enrolled in or willing to enrol in a Certificate IV or Diploma in Youth Work or equivalent prior to commencing work with children)
