Job Description
Under s25 of the Anti-Discrimination Act 1991 (Qld), there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
Are you passionate about helping community make positive lifestyle changes? Do you believe in the power of connection, culture, and health? Diabetes Australia is looking for a compassionate and motivated Health Coach to support Aboriginal and Torres Strait Islander participants in theMy Health For Life(Mh4l) program.
This is atelephone-based coaching rolefocused on empowering people to make sustainable changes in areas like nutrition, physical activity, smoking cessation, and mental wellbeing.
Why This Role Matters
You’ll be guiding participants through a six-session coaching journey, helping them set goals, stay motivated, and celebrate progress. It’s not about telling people what to do—it’s about walking alongside them, listening deeply, helping to educate and encouraging change.
What You’ll Be Doing
Deliver 6 coaching sessions per participant over 6 months
Use motivational interviewing to support behaviour change – this role is about bringing hope, health and healing to our communities
Manage your own calendar with flexibility around family and community commitments
Capture data for reporting and program improvement
Work in a flexible, hybrid model with Tuesday being Team Day in the office
Full time or part time opportunity available
What We’re Looking For
Certificate III (or higher) in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent
Experience working with Aboriginal and Torres Strait Islander communities
Strong communication and empathy skills
Ability to work flexible hours (including one 8pm or two 6pm finishes per week and 4:30pm finish each Friday)
Comfortable using computer systems for data entry and reporting
