Physiotherapist - At Home Aged Care

About the role

We’re seeking a passionate Physiotherapist to deliver evidence-based care to elderly clients under the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), and other funding streams. This is a full-time ongoing role, offering stability and purpose in a supportive team environment.

• Conduct comprehensive assessments and deliver tailored interventions aligned with aged care quality standards

• Collaborate with clients to identify goals and develop care plans

• Provide group-based programs focused on cognition, balance and welness

• Liaise with internal and external stakeholders including My Aged Care, GPs, and case managers

• Maintain accurate documentation and meet billable hour targets

• Travel across aged care planning regions as required

About you

• Tertiary qualification in Physiotherapy and current AHPRA registration

• Strong clinical assessment and report writing skills

• Excellent communication and interpersonal abilities

• Ability to work independently and manage time effectively

• Holds a current driver’s licence and is willing to travel across Metro Melbourne, including the Eastern, Northern, and Western regions.

• Valid Police Check and Australian working rights
