Job Description
Beata Homecare provides home care services to support the independence of clients from various cultural backgrounds in Melbourne and the Bass Coast Region.
We are looking for aCommunity Physiotherapistto join our team to provide in-home physiotherapy services to clients on the Home Care Packages program (HCP), the new Support at Home Program (commencing from 1 July 2025), and NDIS participants, to promote independence, mobility, and quality of life.
You will be based at our office in Mount Waverley, Victoria and will be expected to travel around metropolitan Melbourne and the Bass Coast region.
This is a full-time position. Salary based on experience.
Key Responsibilities:
Perform comprehensive client assessments including mobility, strength, balance, and falls risk.
Design and implement individualised physiotherapy care plans aligned with client goals and program funding models.
Deliver evidence-based interventions such as strength and balance training, falls prevention programs, pain and chronic condition management, mobility and gait re-training, manual handling strategies and training, and assistive technology and home equipment prescription.
Monitor and evaluate client outcomes and adjust interventions as needed.
Collaborate closely with Case Managers, Service Coordinators, RNs, and other health professionals.
For Home Care and Support at Home Clients: Contribute to care planning and review, and deliver services in accordance with Aged Care Quality Standards.
For NDIS participants: Prepare reports for Functional Capacity Assessments and assistive technology, and ensure services align with NDIS plan goals and funding.
Skills & Experience Required:
Tertiary qualification in Physiotherapy (Bachelor or Masters level)
Current registration with AHPRA
Minimum of 1 year post-graduate experience in a relevant setting
Strong knowledge of community care, aged care, and/or disability sector
Familiarity with NDIS Practice Standards, HCP guidelines, and Support at Home Framework
Demonstrated ability to work autonomously and manage a community-based caseload
Excellent communication, organisational, and clinical documentation skills
Other Requirements:
Victorian Drivers Licence and reliable vehicle with insurance
Police Check (within 3 months) or NDIS Workers Screening Check
First Aid Certificate (within 3 years)
CPR Certificate (within 1 year)
Minimum 2 doses of the COVID-19 vaccination
Desirable:
Experience with fall prevention
Manual handling training credentials
Knowledge of chronic disease management and neurological rehabilitation
