About Your Goals Together:
After 5yrs in business we are opening a Clubhouse in Nowra and are looking to expand our services with the addition of more enthusiastic and passionate team members for support worker roles and a small administration role. All experience levels are encouraged to apply.
At Your Goals Together we aim to provide high quality, safe and meaningful services to people living with mental health and physical disabilities through individual and group supports within theShoalhaven Area. This role requires on site attendance within people's homes so please consider how far you are willing and able to travel.

About You:
You are motivated and have a passion for helping others meet their own personal goals. You have a strong understanding and respect for Culture, Diversity and Equality. You display values consistent with a person-centred approach and working in a way that supports the participant’s right to ‘choice and control' and will also have the following:
Certificate III in a relevant discipline (e.g. mental health, youth work, disability, nursing), working towards or equivalent experience.
Current driver license and access to a fully insured and tidy motor vehicle.
Basic computer and smartphone skills.
Ability to work independently and as part of a team.
Knowledge of the NDIS, and NSW Mental Health and Disability system and local services is desirable.
Tasks and Responsibilities
Under the direction of the supervisor, you will be required to:
Provide high quality services for people with intellectual, mental health and/or physical disabilities within their home or the community.
Promote, plan, and engage in meaningful activities that assist people to achieve their goals.
Creative and empowering one on one and group services.
Establish and maintain appropriate and professional work relationships.
Connect participants to the local community with activities and recreational interests.
Support participants with daily living skills.
Encourage and support the development of personal skills through hobbies and interests.
Ensure all participant information, case notes, goals and services are current and regularly maintained and recorded correctly and safety procedures adhered to.
Administration role includes assisting supervisor with roster systems, client intake, file management, networking and marketing.
In addition:
This position will require the successful applicant to obtain a police check, working with children’s check, NDIS worker screening check and to complete NDIS worker orientation module.
This role will require use of your personal vehicle to transport participants. Appropriate award allowance will be payable.
Willingness to obtain First Aid Certificate and Mental Health First Aid.
