What You'll Do:

• Provide personalised active support to individuals with disabilities, focusing on their unique needs and goals.

• Assist with daily living activities, including personal care, attending appointments, meal preparation, and household tasks.

• Encourage and facilitate social and community participation.

• Implement individual support plans in collaboration with participants and their families.

• Foster a safe, positive, and nurturing environment.

• Experience / knowledge in disability and mental health supports

• Able to drive Wheelchair Accessible Van, Loading / unloading wheelchairs in van, attendant controlled wheelchairs – powered and manual
