About the Role
We are seeking skilled and passionateSupport Coordinatorto join our team. In this role, you will assist NDIS participants in coordinating and implementing support services aligned with their NDIS plans. Your work will focus on strengthening participant capacity, improving service connections, and ensuring supports are delivered in a way that promotes choice and control.
Key Responsibilities
Work with participants to understand and implement their NDIS plan using a person-centred approach
Strengthen clients’ capacity to coordinate supports, navigate mainstream and community services, and exercise choice and control
Build collaborative relationships with participants, families, support providers, and NDIA representatives
Monitor budget utilisation and track service delivery to ensure alignment with funding
Prepare progress reports for the NDIA, highlighting outcomes achieved and key success indicators
Ensure services meet required obligations under the NDIS Quality and Safeguards Framework
About You
To succeed in this role, you will bring:
A strong understanding of the NDIS, participant plans, and funding structures
Previous experience supporting people with disability and/or mental health
Excellent communication skills – both verbal and written – with the ability to engage diverse stakeholders
Strong time management and organisational skills, with the ability to work in a KPI-driven environment
Proficiency with technology and documentation (case notes, progress reports, correspondence)
Proven problem-solving and conflict resolution abilities
Creative thinking and initiative in developing support strategies
You must also hold or be willing to obtain:
NDIS Worker Screening Check
Working With Children Check (WWCC)
National Criminal History Check
