About GCLA Gladstone Community Linking Agency (GCLA) has been delivering Beyond Ordinary care and support across the Gladstone region since 1977. As a registered provider of both the NDIS and Aged Care services, we support people to live well at home and in the community.

About the role We’re seeking a compassionate and organisedCare Managerto lead holistic care planning and coordination for older Australians under theSupport at Home Program. You’ll assess needs, create tailored care plans, and ensure supports are delivered with dignity, safety, and respect.
This is a frontline, relationship-focused role that works closely with customers, families, service providers, and internal teams to help older people remain living well at home.
What you’ll do:
Conduct person-centred assessments and develop care plans based on goals and needs
Coordinate services with internal teams, health professionals, and external providers
Monitor and review care plans and customer wellbeing regularly
Maintain accurate records and ensure compliance with aged care standards
Advocate for the rights of older people and respond to concerns or risks
Work collaboratively within the Aged Care team and support new client intake
What you’ll bring:
Certificate III or IV in Aged Care, Community Services, or similar
Experience in community or home care roles
Knowledge of Aged Care Quality Standards and person-centred care planning
Excellent communication, time management, and documentation skills
Police Check, NDIS Worker Screening Check, and QLD Driver’s Licence
Completion (or willingness to complete) SaHP or Aged Care training modules
Desirable:
Experience with CHSP, HCP, My Aged Care referrals, and care planning systems
Cultural competency and trauma-informed practice, especially with First Nations or LGBTIQ+ elders
