We’re Home Based Health (HBH)
A community-first mobile speech pathology service changing the way care is delivered. Our model is built around one simple belief:therapy is best when clinicians are supported, clients are seen in their own homes, and outcomes come from trust — not KPIs.
We work with adults through NDIS, Home Care Packages, Veteran Affairs and private referrals. Our niche? We’re known forleading Parkinson’s carein Melbourne’s southeast — and we're building a neuro-informed hub around it.

What You’ll Be Doing
You'll be supporting adult clients in the community with:
1.
Swallowing, voice and cognitive communication therapy 2.
Home visits and telehealth (clients grouped close to your location) 3.
Documentation and goal-setting using our systems (e.g., Cliniko) 4.
Referrals and care coordination with GPs, carers, and other providers 5.
Low-complexity TAC, DVA, and HCP clients to start – you’ll build confidence fast 6.
Optional: grow into NDIS or Parkinson’s-focused caseloads 7.
Contribute to quality improvement or group program design 8.
Participate in team supervision and one-on-one mentorship 9.
Help shape our emerging neuro hub (if that’s your jam) 10.
Enjoy a manageable caseload and structured admin time
