**About the Role:**Permanent Part Time 32 Hours Per Fortnight
The Grade 1 Social Worker will have consolidated psychosocial assessment and planning skills to deliver clinical services across the inpatient wards and exercises professional judgement to a caseload, adapting routine clinical practice based on valid and reliable evidence.
The successful applicant will support patients and families at Bass Coast Health and use their skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans.
The successful applicant will preferably have a knowledge base in relation to hospital social work supports and services and have an understanding of dealing with State and Commonwealth aged care processes, TCP, GEM at Home, NDIS processes and VCAT and be able to support discharge planning. An understanding of supports for carers as well as care agencies and external stakeholders are imperative in supporting this patient cohort.
Excellent interpersonal skills are essential for this position, including negotiation and communication skills, both written and verbal. An ability to display flexibility within work practice and to work as part of a multi-disciplinary team are also essential.
The successful applicant, positioned within the wider Social Work team, will work in collaboration with their colleagues; the Senior Social Worker, Team Leader and other multidisciplinary staff. They will actively participate in quality improvement and research activities as allocated, participate in supervision, and participate in clinical education programs as required, in collaboration with the Senior Social Worker and Team Leader within the Social Work service.
Key Selection Criteria:
MANDATORY REQUIREMENTS:
Bachelor of Social Work from an accredited course
Membership with, or eligible for membership with, Australian Association of Social Workers (AASW)
A current, full driver’s licence for the State of Victoria and comply with any restrictions on their licence if applicable, e.g., wearing glasses
Employees are required to undertake a National Criminal Record Check, NDIS Worker Screening Check, and a Working with Children Check prior to commencing employment. Noting, employees are required to maintain these checks throughout their employment
Demonstrated experience to person centred practice
Demonstrated ability to work both autonomously and as part of a multidisciplinary team, with demonstrated prioritisation skills
Experience in accessing a variety of community resources and systems
Established skills in organisation, planning and priority setting
Excellent interpersonal and communication skills including proven ability to interact with consumers and their families in a caring and sensitive manner
DESIRABLE REQUIREMENTS:
Clinical experience of the health sector
Demonstrated experience to the maintenance of case notes in client files and hand over notes, and to the timely input of all data entry requirements
Demonstrated knowledge of family violence best practice framework to complete risk assessments and provide appropriate support to patients and their families
An understanding of the principles of continuous quality improvement and demonstrated skills and initiative in service development, evaluation, quality activities and / or research
