About the role
As aService ManageratAffinitycare WA, you will lead and oversee the delivery of high-quality, person-centred disability support services.
Based inOsborne Park, this full-time, permanent position combinesteam leadership, operational oversight, client intake and onboarding, and a strong focus oncompliance and safety.
You’ll be responsible for managing a team of support workers, ensuring efficient service delivery, and supporting clients through a safe and seamless transition into services — all within the framework of the NDIS Practice Standards and Quality Indicators.
What you’ll be doing
Lead, support, and manage a team of disability support workers to deliver tailored, high-quality services
Oversee daily operations, including rostering, service delivery, and ensuring timely, effective client support
Manage intake and onboarding of new clients, including:
Conducting initial intake and risk assessments
Client-service matching
Organising house visits and stakeholder meetings
Ensuring full compliance with NDIS Practice Standards and internal procedures
Liaise with clients, their families, guardians, support coordinators, and allied health professionals to ensure service alignment and continuity
Maintain accurate records and ensure all documentation aligns with audit and quality requirements
Oversee WHS responsibilities, ensuring a safe working environment for staff and clients, including incident management and hazard identification
Provide coaching, supervision, and professional development opportunities for your team
Support continuous improvement by identifying service gaps and implementing solutions
Prepare regular internal reports as required by the Executive Leadership Team
What we’re looking for
Minimum3 years’ experiencein aservice management or operational leadership rolewithin disability or community services
Experience managing support worker teams and coordinating service delivery
Sound knowledge of theNDIS Practice Standards, Quality Indicators, andWHS compliance requirements
Demonstrated ability to lead, mentor, and support teams in dynamic environments
Excellent interpersonal, communication, and organisational skills
Ability to build strong relationships with clients, families, and key stakeholders
Proficiency in client management systems, rostering tools, and digital record keeping
Tertiary qualification in a relevant field (e.g., Nursing, Social Work, Community Services) or equivalent experience
