We are looking for a highly experienced, passionate and caring RN Clinical Coordinator who can demonstrate and deliver the highest standard of clinical care for our clients. The successful candidate will possess excellent work ethic, ability to resolve complex clinical cases and demonstrate they can work under pressure in a close knit clinical team environment, endorsed by AHPRA.
Your Responsibilities:
● ● Clinical and people leadership and the ability to support and lead a multi-disciplinary team. ● Assessment, development and delivery of Clinical Care Plans. ● Thorough understanding Private, NDIS and HCP service contracts. ● Effectively contribute, actively participate and problem solve in Clinical Coordinator meetings, and internal/external Audits. ● Act as a primary point of contact for all programme related enquiries. ● Liaise and maintain effective working relationships with internal and external stakeholders including referrers. ● Organise and facilitate all allied health services for our clients.
●Demonstrate strong leadership and management capabilities.
● Maintain confidentiality and adhere to professional boundaries at all times. ● Utilise clinical experience to assess, evaluate and implement appropriate interventions for care. ● Maintain effective working relationship with care workers and provide training and support where necessary. ● Keep abreast with all relevant external legislation and internal policies and procedures related to the position. ● Promote a strong customer service focus and culture of compliance. ● Maintain up to date, clear and accurate client care plans, records and meet administrative requirements in accordance with policies, procedures and funding guidelines.

Your requirements: ● Current AHPRA Nursing registration (ESSENTIAL). ● Minimum of 2-3 years’ experience as a Registered Nurse. ● Excellent clinical skill base and knowledge of legislation within Aged Care. ● Experience in managing NDIS and HCP clients. ● Friendly and understanding nature for both the clients, guardians, family and friends. ● Experience in leading and supervising teams of people and liaising with families. ● Sound computer skills in Office and Excel. ● High level of interpersonal and communication skills (written and verbal). ● Demonstrated understanding of person-centred care and ability to work in a team environment. ● Current Police Check. ● Drivers Licence.
