ROLE
Arche Health is a not for profit community-based organisation that provides quality support services to primary healthcare professionals as well as delivering quality health care services to the local community.
The Persistent Pain Program (PPP) coordinator will ensure Arche Health’s chronic or persistent pain management in primary care is delivered in an efficient, professional manner and delivered in line with the Primary Health Network funding requirements.
ThePersistent Pain Programis a community basedmultidisciplinary initiativethat offers specialisedAllied Healthsupport in partnership with patients regular GP providing access to assessment and case management of persistent pain. The program consists of multiple components carefully tailored to meet client’s needs. This includes individualclinical facilitationto case manage a patient’s individualisedpain planswith patients’ usualGPand other healthcare teams. This is complemented by a series of individual allied health consultations and face-to-face/online group education.
The program aims to help persistent pain sufferers become better managers of their pain through the expert education program, individual case management, support, goal setting and improved use of community healthcare services. Designed so that participants can explore a range of different strategies for living well despite pain, in the hope of leading to reduced reliance on medication for pain management, reduced requirements for emergency care and living a better quality of life.
We seeking an experienced Program Coordinator for our Persistent Pain Management Program [PPP] located in Bentley WA. The position is Part Time for 3-4 days a week.
RESPONSIBILITIES
Coordinate the programs including administrative tasks, service delivery and evaluation.
Work with multidisciplinary teams to coordinate the program.
Promote the program to all relevant stakeholders.
Follow client’s journey by ensuring receipt of referrals, assessing clients eligibility, booking of clients into group program and individual consultations and collecting data in a systematic manner.
Organise acknowledgement of referrals, mail-outs and progress reports to all referring stakeholders.
Establish formal linkages and referral pathways with key stakeholders e.g. GPs, public hospitals, allied health professionals and local government.
Liaising with referring bodies to utilise team care arrangements.
Organise venue arrangements, groups and individual sessions.
Ensure that all necessary data are collected and processed in a timely manner.
Complete reports on the program to enable targets to be monitored.
Ensuring the program contractual requirements including targets and reporting deadlines are met.
Ensure regular maintenance of program updates in Arche Health’s marketing and social media platforms when possible.
ESSENTIAL SELECTION CRITERIA
Minimum of 2 years of experience of program coordination, preferably working with GPs.
Excellent interpersonal & communication skills
Data Entry & Analysis
Microsoft Suites
Familiar with medical software i.e BP or MD.
Current Police Clearance
