As a registered NDIS provider, we specialise in Support Coordination, empowering participants to achieve their personal goals and enhance their independence. We take a person-centred, co-design approach to planning, implementing, and monitoring NDIS plans, ensuring each participant receives tailored and effective support.
Qualifications & experience
Deep understanding of NDIS Practice Standards, with strong proficiency in adhering to policies and procedures to ensure continuous company compliance.
Highly skilled in interpreting and implementing NDIS plans, with strong expertise in NDIS report writing.
Excellent interpersonal and communication skills.
The right to work in Australia, Working with Vulnerable People Check and / or National Police Records Check and a Working with Children's Check.
Ability to work autonomously, meet KPIs, and efficiently manage a diverse caseload.
A current driver’s license, a comprehensively insured roadworthy motor vehicle and a First Aid Certificate
Proficiency in Microsoft Office applications, and experience with Astalty software will be considered an advantage.
A minimum of 2 years’ experience in Support Coordination is required, and a recognised qualification in disability services would be highly regarded.
Tasks & responsibilities
Building, supporting, and enabling positive relationships with participants and their support networks, and linking participants to mainstream or community services (i.e., housing, education, transport, health).
Support Participants with NDIS Implementation, Progress and Plan Reviews. Also Conducting intake assessments with new participants to identify their needs and requirements.
Developing Support Plans with participants to support the implementation of reasonable and necessary supports, while maintaining the Human Rights of and individual Choice and Control.
Maintain a manageable case load and delivering support coordination within the NDIS Funding Frameworks.
Achieving realistic billable hours against set KPI’s.
Record case notes against participants files and provide regular reports and updates to management on participant issues.
Liaise with NDIA, NDIS Quality and Safeguards Commission, and other key stakeholders as required.
Ongoing review and monitoring of changing NDIS legislation.
Ensure compliance with organisational policy and procedures and that all mandatory reporting is undertaken and required assessments completed within timelines and of a high quality.
Assist with other business functions as requested by the Owner / Business Manager.
