NDIS Support Coordinator / Psychosocial Recovery Coach

We are currently seeking an experienced and motivatedNDIS Support Coordinator / Psychosocial Recovery Coachto join our dedicated team. This is a full-time opportunity for a caring professional who thrives on meaningful work and making a difference.

Key Responsibilities

As NDIS Support Coordinator, you’ll play a vital role in helping participants implement their NDIS plans and connect with the right supports. Your day-to-day will include

Coordinate supports and services in line with each participant's NDIS goals

Build participant capacity to navigate the NDIS and access community resources

Attend plan reviews, reassessments, and AAT tribunals where required

Collaborate with external service providers, clinicians, and families to ensure holistic care

Prepare and maintain accurate case notes, recovery plans, and progress reports

Deliver trauma-informed, strength-based coaching and psychosocial support

Support participants to build coping skills, resilience, and independence

Psychosocial Support and Coaching:

• Provide empathetic and non-judgmental coaching to clients, utilising evidence based techniques and interventions.

• Offer emotional support, active listening, and guidance to individuals facing mental health challenges.

• Help clients develop effective coping mechanisms and resilience skills.

• Create a Recovery Plan and Report and submit it to the NDIS.

What we are looking for:

A genuine passion for helping others

Experience in NDIS Support Coordination

Minimum 2 years of experiencein NDIS support coordination or psychosocial recovery coaching

Qualification in the area.

Sound understanding of NDIS processes and person-centred practice

Excellent interpersonal, communication, and documentation skills

Valid Driver’s Licence and reliable vehicle

Knowledge of community resources and social services.

Empathy, compassion, and a non-judgmental attitude.

Ability to work collaboratively within a multidisciplinary team.

NDIS Worker Screening Check & WWCC
