NDIS Support Coordinator ( Neighbourhood Team Leader)

The Role

Our model of service delivery hinges on thepivotal role of Neighborhood Team Leaders, who act as a bridge connecting clients to their local communities.

Based in Hobart, Tasmiania, working part-time,4 days a week(0.8FTE), you'll lead an existing dynamic Neighborhood Team of Support Workers and manage Support Coordination to ensure our clients receive exceptional person-centered support.

This role would suit an experienced Coordinator looking to broaden their skillset to Team Management. Taking on a number of existing clients and ILO’s, this role would also have a focus of growing the client base in the local community.

**Fully remote but based in the local community,this role will work from home while acting as the beacon of local knowledge for our clients, connecting people to local services. The ideal candidate will be based inHobart or neighboring suburbs.**Your role will include client outreach and growth, providing client support, ensuring quality and safeguarding, and fostering the development of your team.

Key responsibilities include:

Supporting clients to achieve their goals through strength-based and person-centred planning.

Coordinating NDIS packages and fostering meaningful community connections for clients.

Rostering, leading and inspiring a team of local Support Workers to deliver exceptional services.
