NDIS SUPPORT COORDINATOR

About the role

As an NDIS Support Coordinator atYOUR VOICE PTY LTD, you will play a vital role in helping our clients with disabilities achieve their goals and lead more independent lives. Based in our Darra, QLD office, this full-time position will see you coordinate and facilitate the delivery of essential NDIS services to ensure our clients receive the support they need.

What you'll be doing

1.

Developing, implementing and monitoring comprehensive support plans for NDIS participants 2.

Liaising with NDIS providers, therapists and other stakeholders to coordinate services 3.

Advocating for participants to ensure they receive the appropriate level of support 4.

Monitoring participant progress and adjusting support plans as needed 5.

Maintaining detailed case notes and preparing reports for the NDIS 6.

Providing guidance and information to participants and their families 7.

Identifying and addressing any gaps or issues in service delivery

What we're looking for

1.

Relevant tertiary qualification in a allied health, social work or community services field 2.

Minimum 1 year experience in a support coordination or case management role is beneficial 3.

Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients 4.

Strong problem-solving and negotiation skills to advocate effectively on behalf of participants 5.

Demonstrated understanding of the NDIS and ability to navigate the scheme 6.

Passion for making a difference in the lives of people with disabilities 7.
