NDIS Service Coordinator

We are currently looking for a Full TimeNDIS Service Coordinatorto join a small team focused on quality outcomes. You will assist and support NDIS participants, their families and carers to enable them to implement their goals and aspirations.

Working as part of a disability support team theNDIS Service Coordinatorwill maintain an effective and consistent approach to service delivery under the general direction of the NDIS Service Manager. The position requires the person to have a commitment to the aims and objectives of Holistic Horizons Support Services.

Flexible working and a 9 day fortnight (post probation) is available.

Service Coordinator Responsibilities and Duties.

Collaborating with clients, fostering an understanding of their needs and aspirations in order to explore options and develop actions that best meet their care needs;

Liaising with the rostering team regarding client needs/changes

Contact support workers via phone to give a in depth overview of the participant and their care needs before the support worker commences a shift with the participant

Maintaining updated documents where new paperwork has been issued;

Monitoring and ensuring appropriate and timely resolutions of client complaints and issues;

After hours On-call on a rotating roster (roughly a couple of days every 2 months).

About you:

· You are an experienced support worker. You currently hold a minimum of a Cert 3 in Individual Support, Disability or a related discipline or be working towards higher qualifications in Disability or Community Services.

· Previous experience in Service Coordination in disability services.

· A Good understanding of SIL, STA, MIL, Restrictive Practice prevention

· You are adaptable, think on your feet and can problem solve effectively.

· You’re a proactive support worker, who meets all the requirements of that role.
