**Locations:**Rossmore, Austral, Sadlier & more **Support Ratio:**2:1 SIL & 1:1 SIL **Shifts Available:**6am–2pm | 2pm–10pm + Active Nights 10pm–6am Male & Female Staff Needed

About Aligned Community Care
As a full-service NDIS provider, we offer Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Community Support, and Nursing Services. Our mission is to deliver high-quality, person-centred care that promotes independence, dignity, and well-being.
Client Profile
You will be supporting individuals living with a range of mental health and cognitive disabilities, often requiring intensive support. Presenting needs may include:
🧠Diagnoses:
Intellectual Disability
Autism Spectrum Disorder (ASD)
Attention Deficit Hyperactivity Disorder (ADHD)
Post-Traumatic Stress Disorder (PTSD)
Borderline Personality Disorder (BPD)
Other psychosocial disabilities
⚠️Behaviours of Concern:
Self-harm and suicidal ideation
Aggression (verbal and physical)
Property damage
Absconding (leaving the home unsupervised)
Emotional dysregulation
Support involves working underrestrictive practicesand followingpositive behaviour support plans, withmedication administration and monitoringas part of your duties.
Your Role
As a Mental Health Disability Support Worker, you’ll play a vital role in empowering our participants through meaningful daily support, including:
Supporting independent living skills
Managing behaviours of concern using trauma-informed and person-centred approaches
Following and contributing to behaviour support plans
Administering or assisting with medication as per care plans
Collaborating with families, allied health professionals, and stakeholders
Promoting social inclusion and advocating for participant rights
Who We’re Looking For
You are a motivated, compassionate, and experienced support worker who:
Has at least1 year of experiencesupporting people withcomplex behavioursorpsychosocialdisabilities
Holds a Certificate III or IV in Individual Support, Mental Health, or Community Services*(preferred)*
Understands and applies trauma-informed, person-centred, and recovery-oriented care
Has experience with continence care, restrictive practices, and behaviour support
Demonstrates strong communication, documentation, and teamwork skills
