About the Opportunity
The Living Well Consultant is a vital community-based role dedicated to empowering clients and their informal carers to thrive and age well within their communities, with particular focus on those experiencing social isolation, mental health conditions, or self-neglect.
Through comprehensive case management, you will expertly guide clients through complex community and healthcare service networks, working collaboratively to establish and achieve wellness goals that enhance their quality of life.
As a Living Well Consultant, you will ensure clients, and their carers receive coordinated, compassionate support that fosters independence and builds meaningful community connections.
About You
As the Living Well Consultant, you will combine heathcare expertise, case management skills, and cultural competency to advocate effectively for clients while delivering culturally safe, trauma-informed care that respects diverse backgrounds.
Your excellent communication and relationship-building abilities will enable you to build strong rapport with clients and carers from varied communities, while your empathy, resilience, and humility will guide all interactions.
You will demonstrate strong analytical skills to assess complex social situations, develop appropriate interventions, and monitor client progress, all while working effectively under pressure and collaborating seamlessly with multidisciplinary teams and stakeholders.
Essential
Relevant qualifications in the Human/ Behavioural Sciences, Allied Health, Aged Care or Community Services fields
Proven experience in case management experience
Experience working with and supporting informal carers
Minimum 5 years’ work experience within a human services setting
Desirable
Experience working with clients living with mental health conditions
Experience working with clients experiencing severe self-neglect
Understanding of contemporary models of care for managing severe self-neglect.
