About the Role:
We are seeking a Junior Case Manager to join our Riverwood team. You will be responsible for managing a portfolio of clients, assessing needs, coordinating services, maintaining compliance, and ensuring participants receive the highest standard of care tailored to their individual goals and preferences.
Under the guidance of a Senior Case Manager, your responsibilities will include:
Assisting in the assessment of participant needs and supporting the development of individualised care plans and budget;
Coordinating services such as day programs, personal care, medical appointments, and community engagement;
Communicating with participants, families, health professionals, and support workers;
Handling basic participant enquiries, feedback, complaints, and emergencies;
Maintaining up-to-date records in both paper and digital formats (e.g. VC, CRM);
Supporting shift scheduling and time sheet verification;
Helping with participant re-assessments and goal tracking;
Assisting with birthday celebrations, newsletter distribution, and activity planning;
Supporting infection control measures and ensuring PPE protocols are followed;
Performing any other ad hoc duties assigned by your supervisor.
Qualifications:
No prior experience required, but ideally 1–2 years in disability, aged care, or community support;
A positive attitude, willingness to learn, and strong sense of responsibility are essential;
Good communication and interpersonal skills;
Able to manage multiple tasks and work in a fast-paced environment;
Basic computer skills (e.g. Microsoft Office, Outlook);
A valid Australian driver’s license and access to a car is highly desirable;
Possession of (or willingness to obtain):
First Aid Certificate
Working With Children Check
NDIS Worker Screening Check / National Police Check
