**About the Role:**As the In-Home Care Coordinator, you will lead the coordination of tailored services that support clients to live independently and with dignity. You’ll manage assessments, care planning, case management, and team leadership to ensure high-quality, individualised care. This is a key role in delivering responsive, respectful, and culturally sensitive services.
What You’ll Do:
Conduct initial assessments and service reviews to develop personalised care plans.
Provide ongoing case management aligned with funding requirements.
Coordinate referrals and ensure services meet client needs.
Lead and support In-Home Care Carers and Domestic Assistants.
Participate in performance reviews, and audits.
Promote a culture of dignity, respect, and informed choice.
Maintain accurate client data and reporting using care software systems.
Monitor service costs and ensure alignment with individual budgets.
Liaise with external service providers and internal stakeholders.
Participate in on-call duties and provide timely responses to enquiries.
What We’re Looking For:
Strong leadership, communication, and problem-solving skills.
Experience in care coordination and case management.
Ability to work collaboratively and flexibly in a dynamic environment.
Commitment to high-quality customer service and continuous improvement.
Essential Requirements:
AHPRA Registration as an Enrolled Nurse or Registered Nurse.
Current CPR Certificate
Current Queensland Driver’s Licence and access to a safe, reliable vehicle suitable for client transport.
Current vehicle registration and comprehensive third-party insurance.
Police Check
Pre-employment medical assessment.
