Our Opportunity
Are you passionate about helping vulnerable children in the community? Join Cafs, a values-driven organisation as a Home Based Care Practitioner. This role sits within our Home Based Care Team. As a Home Based Care Practitioner, you will monitor and support the placements of children in Kinship and Foster care situations working closely with the carers and providing Case Management.
About You
We are looking for a motivated, values-driven individual with experience in case management and out of home care. The ideal candidate will thrive when working both autonomously and collaboratively within a dynamic team.
Skills & Attributes
- Tertiary qualification in Social work, welfare, community services or similar
- Experience in working with vulnerable young people and case management
- Understanding of Foster and Kinship care arrangements
- Personable and able to build good working relationships/rapport

We provide over 50 programs across a range of service areas providing care for clients within communities across the central highland region.
