This position provides customer based services and involves the admission and assessment of consumers in line with their needs and goals. By partnering and working with consumers we assist in enabling them to safely live in their own home and community.
The successful applicant will be someone who can independently manage and grow a portfolio of HCP clients.
Key Responsibilities include:
Partnering with consumers to develop care plans
Ongoing case management of existing Home Care Package consumers
Proactively promote White Oak to potential consumers and community referral partners (GPs, Social workers, Assessment agencies, hospitals, Pharmacies etc.)
Regular monitoring and monthly reporting on consumer budgets
Working closely with White Oak clinical and allied teams
Scheduling appointments for assessments and reviews
Working with other program and rostering managers to ensure a consistently high level of service is delivered
Keeping up-to-date with government legislated standards, guidelines and reforms
To be success in the role, you will be a well-presented, highly empathetic and compassionate person who prides themselves on delivering exceptional customer service. In addition, you will ideally possess:
Previous experience in Home Care Package Co-ordination
Highly developed communication and organisational skills
Ability to work in a dynamic, fast-paced and changing environment
High level computer literacy (MS Office applications and CMS; preferably Procura)
Kind, caring and respectful of consumers and colleagues identity, culture and diversity.
Essential Criteria
Full-time Availability
NDIS Worker Screening Check (or ability to obtain)
Drivers License
While we do encourage Covid vaccination of our employees, this is not a requirement.
