About the Role:
As an NDIS Support Coordinator at Hera Support, you will play a crucial role in assisting clients to navigate the NDIS system and access the support they need to achieve their goals. You will be a problem solver, capable of working with complex issues and developing innovative solutions that meet the needs of our clients. Your work will be client-centric, and you will collaborate with a variety of stakeholders to ensure a coordinated approach to care.
You will be responsible for:
• Coordinating NDIS plans, ensuring clients access services in a timely and effective manner.
• Working closely with clients/Guardians to understand their unique needs and preferences, while advocating on their behalf.
• Managing and resolving complex issues through innovative thinking and resourcefulness.
• Building and maintaining strong, positive relationships with clients, families, service providers, and other key stakeholders, particularly for those with complex needs
• Offering clear communication at all levels, including negotiation and mediation to achieve optimal outcomes for clients.
• Demonstrate strong written communication skills through the timely preparation of high-quality NDIS documentation and reports.
• Adapting your approach to suit each individual’s communication style and needs, fostering an environment of respect and trust.
• Empowering individuals by providing them with the knowledge and tools they need to make informed decisions about their care and support, while ensuring choice and control
• Demonstrating a solid understanding of NDIS principles, legislation, systems, and processes to ensure compliance and best practice in the support provided.
About You:
We are looking for someone who is passionate about making a positive impact in the lives of people with disabilities and who embodies the values of Innovation, Dignity, and Integrity. The ideal candidate will have:
• At least 1 Full-time equivalent year experience within the disability or human services sector and working in a complex coordination role.
• A proven ability to solve complex problems and implement innovative, resourceful solutions.
• Exceptional communication skills, including the ability to negotiate and mediate with individuals at all levels.
• The ability to adapt your approach to different situations and individuals, ensuring client-centred outcomes.
• Strong organisational skills, with the ability to manage multiple priorities, juggle many concurrent tasks and meet deadlines.
• A solution-focused mindset, with a proactive approach to challenges.
• A commitment to maintaining confidentiality, dignity, and respect in all interactions.
• A strong self-directed learner, who can find out information, be resourceful and learn quickly
• A relevant qualification in Disability Services, Social Work or other allied health field is preferred, but not essential.
