Role Overview:

About the Role Blossom Community Care is seeking highly experienced Disability Support Workers to provide one-on-one care for a participant based in Toorak, VIC. This role requires excellent communication skills, extensive hands-on support experience, and the ability to follow care plans developed by nurses and allied health practitioners. Availability across a flexible roster, including days, nights, weekends, and public holidays, is essential.

Key Responsibilities

Provide high-quality, person-centred support including personal care, daily living assistance, and community access.

Implement and follow instructions from nursing and allied health professionals.

Maintain accurate SIL documentation, compliance records, and detailed shift notes.

Support participant goals and ensure safety, dignity, and confidentiality at all times.

What We’re Looking For

Clear, fluent English communication skills (verbal and written).

Extensive experience as a Disability Support Worker or similar role.

Strong understanding of SIL processes and compliance requirements.

Knowledge and confidence in carrying out directions from health professionals.

ABN required – subcontractor arrangement.

Flexible availability for a range of shifts, including 24/7 care.

Ability to work independently and take initiative.

Essential Requirements

Certificate III or IV in Disability, Aged Care, Individual Support, or equivalent (preferred).

NDIS Worker Screening Check.

Current First Aid and CPR.

Working with Children Check.

Valid Australian driver’s licence and reliable, insured vehicle.

Smartphone with data.
