Disability Support Coordinator

About the Role

We are seeking a compassionate, organised, and proactive Disability Support Coordinator to join our team working part-time for 4 days per week. In this vital role, you’ll be responsible for coordinating supports for NDIS participants, ensuring their needs and goals are met with high-quality, responsive, and tailored services.

Key Responsibilities

Coordinate and implement person-centred support plans in collaboration with NDIS participants, families, and service providers.

Build and maintain strong, respectful relationships with clients and stakeholders.

Ensure supports align with NDIS goals, funding, and best practice standards.

Monitor and review client progress, service effectiveness, and report outcomes.

Provide advocacy and guidance to assist participants in navigating the NDIS system.

Maintain accurate documentation and compliance with NDIS requirements.

About You

Experience in disability services, case management, or support coordination.

Strong understanding of the NDIS framework and support categories.

Exceptional interpersonal and communication skills.

Ability to work independently, manage time effectively, and prioritise tasks.

Commitment to person-centred practice, cultural sensitivity, and inclusivity.

Qualifications & Requirements

Certificate IV or above in Disability, Community Services, Social Work, or related field (or working towards).

Current NDIS Worker Screening Check.

Valid driver’s licence and access to a reliable vehicle.
