About the Role - Community Support Worker**
We are seeking Community Support Workers to join our team, sponsorship is available for the right candidates. Your primary responsibility will be to support NDIS participants living with disabilities in their homes and communities. Available shifts span from Monday to Sunday, 38 hours per week, Full time position.

A Day in the Life of a Community Support Worker**
No two days are the same in this fast-paced and dynamic role. Your daily tasks may include:
Assess the needs, challenges, and goals of individuals, families, and community groups to develop tailored support strategies.

Plan, implement, and manage community-based programs that address issues such as housing, health, education, employment, and social inclusion.

Provide practical and emotional support to clients by connecting them with appropriate services, resources, and advocacy networks.

Liaise and collaborate with service providers, government agencies, non-profit organisations, and stakeholders to enhance service delivery and build community partnerships.

Facilitate workshops, group sessions, and outreach activities to empower individuals and raise awareness of community services.

Promote participation and social inclusion through the coordination of recreational, educational, and cultural programs.

Support vulnerable groups, including the elderly, youth, migrants, and people living with disability, to navigate access to services and overcome social barriers.

Monitor and evaluate the effectiveness of community programs, preparing reports and making recommendations for improvements.

Maintain accurate records and case notes in line with legal, ethical, and organisational requirements.

Advocate for systemic change by identifying service gaps, contributing to policy development, and preparing funding or grant submissions.
About You
We are looking for energetic Community Support Workers who value teamwork and are passionate about helping individuals with various disabilities. Our ideal Support Worker is motivated to explore new activities and hobbies with clients. We welcome applicants of all ages to meet the diverse needs of our clients across the Perth Metro area (North, South, East, and West of the river).
· Desirable Experience: High-intensity support and experience working with clients on the Autism Spectrum.
· Loves to provide quality care, support and guidance to others.
· Seeking sponsorship.
· Have a good attention to detail.
· Exceptional admin and reporting skills.
· Want a career within the Community Support field.
· Willingness to Travel: Flexibility to travel as needed.
