Community Support Facilitator

Are you looking for a Support Facilitator role where you can shine? Riverlink are looking for a superstar Support Facilitator to join our small but dedicated team! This role is suited to someone who has experience in in-home and community supports.

This role will be based in our North Parramatta office, however the geographic footprint for our participants is across Sydney and the role will require some travel to meet with participants and their families/carers.

The successful candidate will be responsible for:

Day-to-day operations of the Community Care Team, while working closely with our Workforce team to ensure our service delivery not only meets but exceeds expectations

Leading a diverse and remote team with the ability to coach and inspire your team

Build and maintain relationships with participants, carers and stakeholders

Work with participants and carers to create bespoke support plans, monitor and review regularly
