ADA Australia is seeking a dedicated Community Connector to join our ADA Link team supporting vulnerable individuals across the South Burnett region, including Nanango, Blackbutt, Kingaroy and Murgon, to access aged care and other essential services.

About the role
As a Community Connector, you’ll provide specialist and intensive support to clients within the care finder target population. Using an assertive outreach approach, you’ll engage with potential clients, local intermediaries, and stakeholders to ensure individuals are informed and empowered to make their own choices.
Key responsibilities include
Delivery of high-quality support through information, support, representation and assertive outreach.
Provision of individual and intensive support to clients to ensure they understand and can access relevant aged care and other services.
Engage with community supports and stakeholders.
Provide high level check-ins with clients on a periodic basis and follow-up support once services have commenced.
Support people to interact with My Aged Care and relevant assessment services.
Network with other organisations to promote ADA Link services.
Maintain high standards of documentation and consultation.
Liaise with other care finder services and support groups for older people.
Forge strategic partnerships with diverse cultural and community groups.
About you
Relevant tertiary qualification or equivalent experience in human services, social work, or community services.
Experience working with older people or disadvantaged/vulnerable groups.
Understanding of and ability to work within a social justice framework.
Ability to understand and apply legislation, policies and procedures.
Proven ability to problem-solve and find solutions for clients.
Excellent written and verbal communication and presentation skills.
High level computer skills – Microsoft Office Suite or equivalent.

Essential requirements
Valid Australian Driver’s licence.
Current (less than three (3) years old) National Criminal History Certificate.
Valid NDIS Worker Screening Clearance.
Valid Working with Children/Blue Card.
