About the Role

We are seeking a passionate and proactiveRegistered Nurseto step into a rewardingClient Services Coordinatorposition. This is a community-based role where you'll work closely with clients, families, and allied health teams to develop support plans, conduct regular visits, and ensure services meet clinical and personal goals.Travel between Sydney and Newcastle for client visits, also some interstate travel.

You’ll be responsible for:

Developing and reviewing client care and support plans

Creating and managing Service Agreements (NDIS)

Providing clinical oversight, reviewing incidents and risks

Supporting client independence and wellbeing

Meeting KPIs for service delivery, satisfaction, and compliance

Travelling to clients across Sydney and Newcastle

What We’re Looking For

-

Registered Nurse (AHPRA) with community care experience

Sound knowledge ofNDIS and Spinal Cord Injuries.

Strong communication and clinical assessment skills

Organised, self-motivated, and confident working autonomously

Full driver’s licence and car, willingness to travel regularly
