About the Role:
ISA’sIntegrated Living Modelsupports young people transitioning from Child Safety into semi-independent, supported living. It’s one of the most rewarding and dynamic areas in the disability sector—and ISA is a recognised leader in this space.
This hands-on leadership role is based at our Kingston Hub. You'll oversee a caseload of participants, lead a team of passionate support workers, and collaborate with ISA’s broader leadership and compliance teams to deliver high-quality, person-centred outcomes.

Key Responsibilities:

Lead, mentor, and support a team of disability support workers
Oversee participant onboarding, ShiftCare rostering, risk management, and daily operations
Review and update ISPs, Risk Assessments, and Emergency Plans
Ensure participant progress is goal-aligned, culturally responsive, and strengths-based
Collaborate with families, guardians, plan managers, and external stakeholders
Monitor compliance with the NDIS Practice Standards and reporting obligations
Respond to incidents and support behaviour strategies with a trauma-informed approach
Contribute to ISA’s culture of accountability, structure, and participant-centred excellence
What You Bring:

Proven experience in the NDIS, youth, mental health or community sector
Leadership or coordination background (Team Leader, Senior Support Worker, Coordinator)
Deep understanding of safeguarding, risk, incident management, and support planning
Confidence using digital systems (ShiftCare, SharePoint, Elmo or similar)
Strong professional boundaries, written communication, and emotional intelligence
Cert IV or higher in Disability, Community Services, Youth Work or Mental Health
Current Blue Card, NDIS Worker Screening, First Aid & CPR, and driver’s licence
