The Opportunity
Uralla Shire Council is seeking a compassionate and experienced Care Coordinator to join our Community Care team. In this rewarding role, you will work closely with older people and individuals with disabilities to develop and implement person-centred care plans that support their independence and wellbeing at home. You’ll coordinate services, manage budgets, and contribute to the ongoing development of inclusive, high-quality community care programs. This is a permanent position subject to ongoing Commonwealth and State funding.
The Candidate
You are a dedicated professional with a qualifications in Case Management or a related field, and experience in the aged and/or disability sector. You bring strong case management skills, a deep understanding of community care practices, and the ability to work both independently and collaboratively. Your communication and interpersonal skills allow you to build trust with consumers, families, and service providers. You are adaptable, ethical, and committed to continuous improvement, with a current driver’s licence and First Aid certification.

The position is classified asGrade 5 – Band 3 Level 1under theLocal Government (State) Award 2023, with a commencing weekly rate of**$1,482.30**.
