About your new role
The Department of Health and Aged Care (DOH) has funded the Aged Care Navigator Program (ACNP) as part of its commitment to support older carers and family carers of frail aged individuals. Delivered through the Carer Gateway, the ACNP helps carers effectively navigate the often complex My Aged Care system, ensuring they can access the appropriate services and supports for themselves and the person they care for.
Wellways Australia, a lead Carer Gateway service provider, has been funded to deliver the ACNP across Queensland, South Western Sydney, and the Nepean Blue Mountains regions. The Aged Care Navigator role is a 12-month position (with potential for extension) embedded within the Carer Gateway Services program. Introduced in 2020 as part of national aged care and carer reforms, the Carer Gateway Services program represents a significant shift toward innovative, person-centred support models for unpaid carers. This role plays a critical part in assisting carers to understand and access aged care services while reducing stress and improving outcomes for both carers and those they support.
You will need:
- Tertiary qualifications in a related field (e.g. social work, human services, aged care, community services or health) and/or demonstrated relevant experience
- Current Working with Children Check and NDIS Worker Screening check
- Current Drivers Licence
- Satisfactory National Police Records Check
- Right to Work in Australia
- NDIS Worker Orientation Module Certificate
